When the user submits their form or survey, they are shown a summary of their answers and asked to check them before proceeding to the next step.
If it's a very simple form or survey then you can probably answer 'No'

Yes: When the user submits their form, the system will show a User Details page where they will fill out their Name, email address etc.
No: The User Details page will not appear. You will need to include all the relevant questions in the form yourself.

How many options can the user choose from?

Minimum: 2. Maximum 10. If you need more than 10, you could break them up into two separate questions where one of the answers to the first question is "None of these"

What you type here will appear on the screen to prompt them to enter it:
Does the total price change if they choose this option?
By how much? This amount will be added to the Base Price that you set in the top row of this form/product.

For example, let's say you have a question in the next row of your form. But, that question isn't relevant if they choose this option. By choosing 'Yes' the question in the next row will be hidden if they choose this option.

Select this for the 'No' option if you are asking something like "do you accept the terms and conditions. Or, "You must be a student to enter. Are you a student?"
